It is actually a bug that can be cause by different things. Sometimes it’s when you use the disc in water, a specific perk combination or just a display bug for the Fireteam.
In some occasions the Predator where not aware of this situation since on their screens everything is normal. Some others are aware of it and use it to their advantage.
I never experienced that myself. I only had one Pred that was perma cloaked in all the time I was playing.

It’s been around since at least patch 2.11 and @IllFonic continues to remain quite about it.
It’s a bug/exploit and some people have even figured out how to trigger it.
