2FT members dmg skips second wind?

So ive been in second wind once against a team, but the second time around it totally skipped it with only 2 FT members and Pred having at least 2 meds left!?
no second wind occured not even for a split second. I automatically went dead…no -

This has happened to me once on ps4. Long time ago.
