Alarms not being triggered

So i have had it happen numerous times now that if you kill a npc or run without cloak through a place so they hit the alarm they actually dont sound the alarm. I even killed a npc infront of a other npc and nothing happens.

yeah iv also had it where they sound the allarm but then no one shows up lol.
