Echos of an explosion and exponentially growing in loudness. I think it might have something to do with grenades exploding on Derailed West area where there are high elevations and a constant echo. Eventually it stops but my speakers are blown!
Sometime the reverb in the game is exceptionally apparent in some areas. So much so that it sounds utterly over used.
Audio for explosions are repeating sometimes!

I have experienced the same thing on all 3 maps, no idea what is causing it.
