i managed to regrow mask with disk more consistently than the one shot combi bug but it had to be done in privates to get it easier
i saw many players even on ps do it to get mask back

I’ve had the combi do damage as if it hit me twice, but I’ve also had it down me in one hit, or kill me in one hit, from full HP 175 with OWLF Training.
is erratic, just like the reckless/fanatic bug was dependent on the amount of damage you took before being revived

You provided the evidence yourself. It was from your POV, not the Predator’s. So there is no excuse like there was a bug in the cloak, a blood splatter, or that he moved—nothing like that. The Predator jumped onto the roof, while you had to use the stairs to get up there, meaning you lost sight of him, and he wasn’t spotted. Yet, you ran exactly to the place where he stood and stopped 10 meters away from him. You waited about five seconds, got nervous that he might use the disc or something, and then shot at a completely still and cloaked Predator—directly in the head. You didn’t even try to pretend you were shooting randomly, like you heard him clicking and were spraying at the source of the sound.
