How “wounded” do you have to be for the fanatic specialization to kick in and provide the speed and damage boost?

Isn’t it just losing like 1 health and you’re good for the whole game or do you have to go down to a little bit less than half then you’re good for the whole game?
That’s what I’m wondering
50 hp exactly also for some reason if you heal with a syrrette the buff doesn’t go away but it will go away if you use a medkit
it’s probably a bug just like enraged never goes away once proc’d
