Hmm… I don’t own a PS so haven’t gotten that version myself.
But when I try looking for it I can’t find that it includes those things except for the Deluxe Edition.
But I guess I could be missing something.

The avatars and theme were mentioned on the game page on PS Store when I bought it. They might be PS Store exclusives. Physical versions might not come with them.
I found this:
“People purchasing the game via the PlayStation Store will also receive eleven Predator avatars and a dynamic theme.”
Hmm… haven’t seen that before.
Could find anything of that on the Playstation store itself.
Only mentioned the Theme with the Deluxe Edition.
Nontheless, seems I must’ve missed some information.

I pre ordered the standard edition on the Gamestop website and received the physical game in the mail.
So basically the people who get the avatars and themes are the people who pre ordered the game on the PS Store for a Digital download of the game?
