There is a mission to “kill the surgeon” and every game I have played the surgeon never spawns and we have to either wait the time out or kill the Predator.

I had that same issue, I think the surgeon just gets assigned to some random NPC, regardless of whether or not they are already dead.
I noticed that the pip has taken me to dead NPCs several times.
