thrown spear does not hit on reticle

just had a game where i had the dot on someones head and he threw it completely to the left… whats up with that?

its like a good foot to the left. 2nd time i may be 5 feet from FT trapped in bear trap, reticle is on FT and thrown spear is completely off to the left.
