I’ve done the achievement multiple times on all maps with all varying loadouts so I can’t give specific information as to what may be causing the trophy to not unlock. I’ve played all matches (while trying to unlock the trophy) with my wristblades ONLY. There’s been matches were I stay on the ground, don’t claim trophies, never cloak, and when a FT member is down I continue to wristblade them. I’ve also done the achievement without healing myself and WITH healing myself.
